Quite fortunate to have that unsafe release decision reversed, but then I’ve never seen why it should be the driver that’s penalised.

Who else would you penalise?
If you penalise the team then the only penalty would be a meaningless fine, then there would be unsafe releases everywhere if it meant a place on the track.
Manufacturer points deduction?
That is even more draconian than a driver time penalty, not sure the crime fits that level of sentence.
Penalising the driver does in a way penalise the ‘team within the team’ as it will be the drivers side of the garage who made the error, same as say a bad pit stop.
