Finally worked it out.
The LG recognises Dolby Vision and then gives a bespoke set of picture options for that format.
It defaults to Cinema Home (ISF is just for 4K) which is the recommend setting as it gives most tweaking scope (on the informed guides).
I drilled in to the settings and found TrueMotion was set to “low”. I had turned this off completely, but have now found that you need to do it for any variation of Pixel source type.
That’s what was doing it, as it’s the soap opera effect setting.
I thought turning it off was turning it off!
I use this guide which is comparable to most online for my TV

It’s a quagmire of settings and different ones for different types of HD.
Also, all my sources go to my AV amp and one HDMI to my tv.
Which means I can’t change my telly to “game” for PS4 without manually doing it (otherwise I’d ascribe a set HDMI).
I get why a lot of people give up and just stick to factory settings.
